ABSTRACT

Objective To investigate the diagnostic value of one-stop liver magnetic resonance dynamic enhanced MRA for esophageal and gastric varces. Methods 82 patients with liver cirrhosis portal hypertension who were diagnosed by clinical and laboratory detect were taken, conducting liver horizontal scanning and multitemporal three dimensional dynamic enhanced scanning using Philips Achieva 1.5T MRI scanner, and then make maximum intensity projection(MIP) at work station to draw up portal vein image, observing the form and distribution of portal vein and its branches, meanwhile refer to cross sectional image to carefully observe whether there was cirsoid vein in esophagus and stomach, and further the source, degree and depth of cirsoid vein were compared, and whether there was cirsoid vein in other parts, and the pathological situation of liver.Compared with the result of gastroscope inspection, whether there is cirsoid vein in esophagus and stomach. Results The trunk and class-Ⅲ branches of portal vein of all patients had been clearly shown. In this team, there were esophageal and gastric varces in 78 patients, specifically including varices of fundus of stomach & esophageal varices in 27 patients, and varices of fundus of stomach and varices of body of stomach in 19 patients.Meanwhile,it diagnose 6 patients with liver cirrhosis and liver cancer and indicate 8 patients with liver cirrhosis nodule cancerization. The endoscope found out esophageal varices and varices of fundus of stomach in 68 patients, specifically including gastric varicesa and esophageal varices in 25 patients, sole gastric varicesa in 8 patients, and sole esophageal varices in 35 patients. Conclusion The one-stop liver three dimensional dynamic enhanced MRA is with great value in diagnosing the location and degree of esophageal varices and varices of fundus of stomach of patients with portal hypertension. The overall effect is better than endoscope.

ABSTRACT

Objective To investigate the imaging techniques of enhanced T1 high resolution isotropic volume examination (e-THRIVE) multi-phase dynamic contrast-enhanced scanning technology. Methods 83 patients with liver diseases underwent routine and contrast-enhanced MR scanning.e-THRIVE axial volume scan was used in contrast enhanced MRI.The raw data acquired were post-processed with maximum intensity projection(MIP) technique, and to observe the esophageal and gastric varicose vein's display in different injection rates of contrast agent with delayed scan time. Results The varciation of esophageal and gastric blood vessels in 79 cases were detected, the MIP images showed the overall effect of varicose blood vessels, and the axial images displayed that varicose blood vessels' location,in the submucosal or the inside or outside of the lumens. The ratio of high-quality image reached 72%.Conclusion The high-quality vascular imaging technology of e-THRIVE depends on appropriate injection rate and the right amount of contrast agent, the best delayed scanning time and patients with good breath-hold.

ABSTRACT

Objective To study the clinical significance of magnetic resonance imaging in evaluation of diagnosis and treatment of tibia plateau fractures accompanying with knee joint injures. Methods From July 2000 to November 2005,52 cases of tibia plateau fractures were examined by X-ray and Magnetic resonance imaging(MRI),which could accurately diagnose the splitting and depression degree of tibia plateau fractures and the knee joint injures. Results There were 52 cases of tibia plateau fractures, including 14 cases lateral plateau fractures,11 cases medial plateau fractures,11 cases total plateau fractures,18 cases tear-off fractures of tibia.MRI could accurately display fractures accompanying with knee joint injures, including fragmentation of catilago articularis, lancination and displacement of meniscus, lancination of collateral.According to MRI displacement direction of tibia plateau fractures and areas of depression could be accurately evaluated before operations. Conclusion MRI can accurately diagnose the feature of tibia plateau fractures and knee joint injuries. And according to the scope of fractures, the best treatment methods can be chosen.

ABSTRACT

Objective To explore the clinical characteristics and causes of misdiagnosis of primary small intestinal tumors. Methods 54 cases of primary small intestinal tumors were retrospectively analyzed. Results Among them,32 of 54 cases were misdiagnosed for 2 to 16 months.Preoperative misdiagnosis rate was 59%.In this group,11 cases were benign tumors while the other 21 cases were malignant tumors located at duodenum jejunum and ileum were found in 7 cases,13 cases and 12 cases respectively. The most common clinical presentations were abdominal pain, abdomen mass, gastrointestinal haemorrhage and jaundice, etc. The accurate diagnosis rate of whole digestive tract radiography and CT scanning was lower, while small radiography of hypotonic and CTVE and MRVE and DSA was helpful to make sure the portion and quality of the lesion. Conclusion The reasons of misdiagnosis are following:insufficient understanding about the disease, the clinical presentation of primary small intestinal tumor is untypical prognosis of these patients.

ABSTRACT

Objective To investigate the manifestations of MRI for congenital coarctation of aorta. Methods Twenty-five patients with coarctation of aorta were studied, including 11 cases of simple type and 14 cases of complex type. All of them were examined by transthoracic echocardiography and MRI including SE sequence, Cine MRA,3 dimension dynamic contrast enhanced magnetic resonance angiography scanning. Results Among 25 cases of coarctation of aorta, MRI could directly show the whole picture of abnormality, including the scop, degree and kinds, obviously better than diagnosis by trimsthe rack echocardiography, according to operation. The accuracy of diagnosis for congenital coarctation of aorta and complicated malformation of congenital coarctation aorta was respectively 100% and 78% .Conclusion MRI examination for coarctation of aorta is important not only for diagnosis but also for the treatment and follow up,MRI can clearly show the kinds, scope and degree of congenital basically replacey X-ray angiography.

ABSTRACT

Objective To explore the value of MRI features of the appendixes bone in differentiating vertebral myeloma from metastatic tumor. Methods There were vertebral myelomas and metastatic tumors in 32 cases respectively involved the appendixes bones, the number, positions, forms, size, distribution ranger and T_1 of the lesions, the vertebral pedicles and the diameters of the cross sections of vertebrallaminas were analysed. Results Among the 32 cases of vertebral myeloma, the destruction of appendixe was bilateral in 21 cases andunilateral in 11 cases.Totally 89 lesions were checked out ,of them, the lesions were the form of mass or nodule in 39,patch in 28,mixed form in 12.In compared with its adjacent and normal one,the ratio of evary invaded vertebral pedide , vertebral lamina and spinous process was between 1.048~2.135 . There was significant difference (P0.05).Conclusion The difference of MRI features of the appendixes bone invaded by vertebral myeloma and by metastatic tumor is helpful in distinguishing vertebral myeloma from metastatic tumor.
